Hi I have been suffering for some years with this abominable pain. I have tried most of the systemic drugs , I asked specialists and Doctors if I could at least try a topical treatment but because this requires a special prescription have been refused Has anybody had a chance of trying these? Thank you I will try to put a link on to some of the research into Gabapentin Gel. Thanks.

Is it just me or is it common for women with pelvic floor dysfunction to clench the muscles of the butt all the time, I actually have the consciously tell them to relax. Interested to see if this happens to anyone else

It's VERY common in women with sacroiliac joint dysfunction, have you had those checked? It is something some women do with stress too so it could be that. I do it all the time, my pelvis is very unstable so my tight pelvic floor is part of my brains effort to help but isn't enough so the brain searches for more muscles to recruit for more stability and the glutes are one of the obvious choices as they are technically a stabiliser as well as a mobiliser but gripping with them isn't how that's supposed to work so it causes more dysfunction. If you haven't had your pelvic stability checked I'd advise getting it done, it usually involves laying down and being asked to lift a leg at a time while they monitor your pelvis for movements and one leg standing where they monitor the ilium and sacrum to see how they move in relation to each other. Any physio should be able to check that but a Women's Health Physio will know more about it. If you clench the glutes they will also be weak and full of trigger points (much the same as the pelvic floor in pelvic floor dysfunction) so you should invest in a decent trigger point ball to work on those. The clenching can be consciously stopped but if instability is what's behind it the correct muscles (transversus abs and multifidus) will need to be worked on to replace the faulty stabilisation from the glutes. Clenching with the glutes also restricts the hips so your spine has to do all the flexion movements so it really is bad for us in loads of different ways!

Thanks for the info Sarah! I do currently see a women's health Physio but she hasn't done anything to check all the other muscles supporting the pelvis, just my pelvic floor muscles...i will ask her to check it over. I am seeing a chiro and a massage therapist who specialises in myofasical massage. I am seeing a Chiro who has said my pelvis is twisted and after 5 sessions, she can't make the adjustments stick which she said is unusual, so that is why I started seeing a massage therapist to look at all the muscles and attempt to release them and stop them pulling at my pelvis. The massage therapist said my glutes are weak as well. It is funny though, i don't get any back pain currently, even though the muscles are all screwed surround my pelvis! Guess that will change as I get older, my spine can only cope with so much!

I had muscle issues likes that too but as soon as I got the surgery the muscle pain went away. Idk if that's an option for you but it worked for me. The only thing that still hurts is my hip but I was im an accident a few years ago and it'll never be right. I did pt but honestly that always made me feel worse because of the affected skin/nerves. I really feel that if you're in pain day in day out your muscles start compensating and you begin to hold yourself different putting even more strain on your body. Just a theory. I go for my 6 week appointment today so hopefully I know more but my muscles honestly stopped bugging me a few weeks after.

cba321 if your chiro can't make the adjustments stay put you have pelvic instability, you need to work on your core strength (minus the pelvic floor) to stabilise things. The massage therapist needs to be careful not to release anything that is trying to stabilise you or it will make matters worse so if it's your right side that won't stay put (it usually is) have her avoid alot of that side and treat the other side which will be tighter and more stable (one side is always more stable than the other). Your pelvic floor is likely doing what mine is and attempting to stabilise you so it might take more stability first before your PF will release, your WH physio needs to know all about what the chiro is doing and where you can't stabilise. And no offence to Alana but surgery will not help when you have pelvic instability issues! If you want to chat more feel free I know tonnes about pelvic instability after suffering it for 8 years and counting!!

No, it wont but it made it significantly better for me. But that's me, everyone's different, so I don't see how you can say it won't make a difference. I still have muscle issues, but with the surgery, they have significantly decreased because I don't have constant pain. I have no idea what your treatments are like, but for me this worked, so it was just a suggestion. And obviously it won't work for everyone... I still see a PT specialist, but at least its better.

Calm down Alana, my point is PELVIC INSTABILITY can't be helped by having the surgery not that nobody can be helped by it. Pelvic instability is caused by the pelvis not being stable and muscles hanging on to stabilise it which wouldn't be helped by a vestibulectomy, do you see what I mean? If your surgery has worked I'm very pleased for you but pelvic instability relates to alot more than just the pelvic floor and vulvar so needs a different treatment and if your surgery has worked then pelvic instability obviously wasn't your problem. We're at cross purposes here I think because we're discussing different causes of vulvar pain.

I'm calm, thanks, just pointing out that different things can cause it to diminish that's it. I don't know what pelvic instability is, but it wasn't mentioned in the first post, all I was doing was adding my opinion to the first post. But yeah, I get it, I'm over this whole deal, it sucks
