Hi I have been suffering for some years with this abominable pain. I have tried most of the systemic drugs , I asked specialists and Doctors if I could at least try a topical treatment but because this requires a special prescription have been refused Has anybody had a chance of trying these? Thank you I will try to put a link on to some of the research into Gabapentin Gel. Thanks.

Hey everyone,

I've had provoked vestibulitis for my whole life, but obviously didn't really notice it until I started having sex at age 21 (I'm 25 now). Over the next couple years the pain came and went, though I think it was mostly present. There was randomly a magical 6-month period where it was almost non-existent! Other than sex I also have really extreme sensitivities to things like latex and semen. If those things come in contact with my vagina, it feels like the worst burning ever, sometimes I almost vomit.

I just left a negative relationship, quit my job, and moved back across the country to live with my parents so that I could finally get this sorted out. It's been more than 6 months since I attempted having sex - last time, it felt like burning, stabbing knives. This entire past 6 months or so has been extremely stressful, and my body definitely shows that stress...all of my pelvic floor muscles, which were always tight, are still extremely tight, and the pain is now radiating out to my lower back, legs, and shoulders. I feel achey and fatigued all the time.

I've been doing pelvic floor physical therapy and working with dilators. This has been somewhat helpful, though it isn't helping the vaginal pain at all. The only change I can see is that it's sometimes easier to poop, which before was very difficult. Sex for me was always a situation where once the penis was in, it was mostly fine...it was the initial penetration that was the painful part, thus the vestibulitis. The pain is almost completely centered around the bottom part of my vagina, and is very intense in one spot.

My question is...is physical therapy really going to help this? I see that pain as the root of all of this muscle clenching, so should I be doing something else to help that pain as well? Physical therapy doesn't really seem to be related to that...

Your situation sounds like mine- I was doing physical therapy, topicals etc etc., but nothing felt better. Finally, I went to a vulvodynia specialist and he said that the physical therapy was helping, but that the part where I was reacting to the most was nerves and skin that was inflammed and misfiring (basically, he used different words). So I decided to go ahead with the surgery, got it done, and now I can use my vibrator without any problems, except initally because my muscles are so tight down there! I will tell you, that I still do get burning down there, but its mostly muscle issues as soon as I get my muscles relaxed, the pain goes away. I was like you (I haven't had sex in over a year so don't ask how sex is postop because I haven't got a clue lol), as soon as the penis got in, I was fine, but that initial part was like fire in my hooha! I think the physical therapy will help your muscles (vaginismus) but not the sore part of your skin (vulovydnia). Where do you live? And have you tried anything else for it? Don't discredit it though, my physical therapist got me having sex with vulvodynia, and it wasn't nearly as bad as before, but because the skin was still inflammed, it still hurt. Now, it's just muscle pain, and he wants me to do botox, I don't know how I feel about that. Let me know if you have any other questions! Very Happy

Hey Alana!

Your thread with Jen about your surgery was what got me thinking about this! I had always completely ruled surgery out, but as I continue with my physical therapy, I'm noticing that while my muscles are reacting well and progressing, that one little spot of nerve pain is still SO painful. And if someone could just, well, cut it out, I could probably be fine! Thankfully, I live in Portland, OR and apparently we have great vulvodynia surgeons here! So it's possibly an option. However, I'm going to give it more time and work to continue reducing stress in other areas of my life which will hopefully help with all the pain. I would love to get to a place where I weren't clenching my pelvic muscles all the time.

A few questions:
- Did you ever try a sort of vibrating dilator along with your other dilators? I've heard that this can help strengthen skin and make it less sensitive/itchy etc. I just ordered one and I'm curious if it will help.
- What was your physical therapist having you do? Mine just does a few stretches on each side of the vagina every appointment which definitely makes an impact, but perhaps there is more to do. I also do yoga and some general stretches and pelvic-specific stretches everyday on my own.
- Did your back, thigh, and hip muscles ever become extremely tight? Mine are out of control right now and the back pain is probably the worst part of this for me at the moment. I wonder if it is contributing to the nerve pain at all...
- Did you deal with vaginal dryness at all? What did you find helpful? I have used things like Vitamin E oil and seabuckthorn seed oil and those were somewhat helpful but I'd be open to more suggestions.
- Did you ever have pain in your clitoris? I've been having dryness and pain and hyper-sensitivity lately which has been really uncomfortable. Would love any advice about that.

I guess that's it for now...it's nice to find someone who has what I have as opposed to the constant burning (which sounds HORRIFIC by the way, I feel so bad for anyone who has that). I had a really negative experience the other night where I tried to go up to a bigger size of dilator and it hurt SO BAD and brought back a lot of memories of painful sex which felt really unpleasant and set me off on feeling negative for days. Hard to stay positive sometimes but your story has me hopeful!

OMG the surgery was the best thing that ever happened to me. Don't rule it outcomplteely, it was worth it! This is gonna be really quick, sorry let me know if you have any questions after you read this, I have an appointment lol



I don't have dialators I use my vibrator for that Wink

I do yoga too, but breathing stretches has helped me too- ask her if she has any for you to do on your own. I was clenching my muscles too, but I exhale my breath, and stick my stomach out and it unclenches and feels good too- and what's best is you can do it and no one will even know!

My every muscle is tight so yes I have that too lol

And no to vag dryness lol I have the opposite problem, but I am not complaining and no clit pain, unless someone jammed me there lol



With the dilators I go a little at a time and use LOTS OF LUBE! and when I get to a painful point, I rest it there and let it relax! Don't jam anything up all the way because that will be painful. But yeah the surgery was the greatest thing going for me (not the most fun experience, but hey whatever its over now!). One of my best friends lives in Gresham, is that near you?

Hi Raquell-

Just wanted to give you some feedback on my physical therapy experiences. I started out for a few weeks with my first physical therapist who focused more on the surrounding muscles. Like you, I felt like it was (maybe?) helping, but was a roundabout way to get the root of my problem which is a burning pain at the vaginal opening. Anyway, that therapist got sick for 2 weeks and then went on vacation for a week so I was put with a different therapist who has a completely different approach. This lady goes right in for the goods...she basically rubs her finger with olive oil in little circles right where the pain is. Hurts like a bitch, but we push on through until the pain starts to go away. It's weird because the pain is absolutely excruciating when she first starts, but then becomes tolerable after a few minutes. Once I reach the point where it feels ok, she moves on to a different area. She said this technique basically causes the nerve endings to just "let go" in simple terms.

I've been with this new therapist for 3 weeks now and am starting to see some improvement. I'm going to try some steroid injections tomorrow and will continue the therapy afterward so hopefully this nightmare will end.
